Medication reviews for people living with HIV

People living with HIV are being better supported following a hospital stay, thanks to the Integrated Sexual Health Pharmacy Team at Kent Community Health NHS Foundation Trust.

While in hospital, medications can often be changed and the team recognised the importance of a medication review soon after someone is discharged. They aimed to increase the amount of patients receiving a medication review following discharge from 42 per cent to 75 per cent, but managed to do this for all during their quality improvement project.
