Changes have been made to increase admissions to community hospitals directly from home.
A team at Kent Community Health NHS Foundation Trust aimed to increase rates from an average of 2.8 per cent to five a month, but during their quality improvement project they raised this to 6.1 per cent, making things better for both patients and colleagues.
Before the changes, community teams found they sometimes struggled to admit patients from home to community hospitals. As a result, some patients ended up in acute hospitals, when community hospitals could have met their needs.
